New Patient Registration

registrationWe accept new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a registration from and health questionnaire. 

Medical treatment is available from the date of registration (please give 48hrs from time completed paperwork is received).

To Register at the Surgery

To register at the surgery, you will need to attend the surgery with the following information:

Adults:

  • Photographic ID - Passport/Driving Licence (original no photocopies) & 2 x Utility Bill - showing your name & new address
  • Birth Certificate (original no photocopies)
  • NHS Number (where possible)

Child over the age of 5:

  • Birth Certificate or Passport (original no photocopies)
  • NHS Number (where possible)

New Baby or Child under the age of 5:

  • Child Red Book
  • Birth Certificate or Passport (original no photocopies)

When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

If you have any queries prior to attending the surgery please contact reception.
